Hypothyroidism and Its Types

Hypothyroidism is a syndrome caused by long-term deficiency of thyroid hormones in the blood or their reduced biological effect at the tissue level, leading to disruptions in all metabolic processes in the body.

This condition has a broad classification. First of all, it can be either congenital or acquired.

Depending on its origin, hypothyroidism is categorized as:

  • Primary (thyreogenic) – caused by problems directly in the thyroid gland;
  • Secondary (pituitary) – due to pituitary dysfunction;
  • Tertiary (hypothalamic) – related to hypothalamus disorders;
  • Tissue (transport, peripheral) – when hormone transportation or cellular sensitivity is impaired.

Among these, the most common form is primary hypothyroidism, often caused by Hashimoto's thyroiditis — an autoimmune inflammation of the thyroid gland. Depending on disease progression, primary hypothyroidism is classified into stages:

  • Latent (subclinical);
  • Manifest stage;
  • Compensated;
  • Decompensated;
  • Severe (complicated) form.

Subclinical hypothyroidism is the most common type and often goes unnoticed. It can only be detected through blood tests for thyroid hormones. The key sign is elevated levels of thyroid-stimulating hormone (TSH), which regulates thyroid hormone secretion.

Complications include myxedema (in adults), cretinism (in children), heart failure, and even hypothyroid coma.

Causes of Hypothyroidism

In addition to the above environmental factors, hypothyroidism may develop due to:

  • Medication side effects;
  • Inflammatory processes in the thyroid gland;
  • Radiation exposure or surgical removal of the thyroid gland.

Congenital hypothyroidism is usually caused by aplasia or dysplasia of the thyroid gland, impaired hormone biosynthesis due to genetic defects, or maternal autoimmune thyroid disease during pregnancy.

Secondary hypothyroidism develops as a result of pituitary damage — such as tumors, congenital defects, neuroinfections, or head injuries.

Symptoms of the Disease

Hypothyroidism affects practically every organ system. Visually noticeable signs include:

  • Swelling and yellowish skin tone;
  • Swollen vocal cords and hoarseness;
  • Hair and eyebrow loss;
  • Dry skin;
  • Disturbed function of sebaceous and sweat glands;
  • Slowed speech and facial expressions.

Internal symptoms include memory loss, development of hypertension and bradycardia, pulmonary dysfunction, digestive issues, and nervous system disorders.

All listed symptoms manifest differently depending on age. They are most clearly expressed in young and middle-aged patients. In elderly people, the signs are often blurred and mainly appear as cardiovascular problems. In children, symptoms depend on the timing of the disease onset.

Diagnosis

Monitoring and early detection of hypothyroidism involve clinical methods — urine tests, complete blood count, biochemical blood analysis, hormone testing, and immunological tests. Radiological examinations and CT scans are also used when necessary.
